About the CTG Project

The Community Transformation Grant (CTG) Project, funded by the Centers for Disease Control and Prevention (CDC), gives communities an important opportunity to develop and implement initiatives to reduce risk factors responsible for chronic diseases, the leading causes of death and disability.

As the nation's public health agency, CDC's mission is to promote health and quality of life by preventing and controlling disease, injury, and disability. To that end, one of CDC's five priorities is to advance evidence-based health policies.

The goal of CTG Project is to create healthier communities, thereby transforming the nation, by making healthier living easier and more affordable where people work, live, learn, and play.
